Job Description
Develops and implements a system to collect and maintain organizational partner relationships.
Works as a liaison between the Bureau of Community Partnerships and Health Equity (BCPHE) Director, Program Manager, and LDH offices to improve communications for those that are non-English proficient.
Develops strategic plans to address various identified barriers to equitable health outcomes.
Works to become a subject matter expert on population health, equity, and its intersection with state health systems.
Supports the collection, review, analysis, and report development on state health equity and community partnership activities.
Supports external efforts to ensure the department’s health equity lens focus is addressed within all policies, programs, and funding decisions within LDH.
Assists in developing and facilitating multi-sector collaborations to address identified community health issues.
Conducts research on population health needs, data and outcomes.
Conducts research on disparate health outcomes, the social determinants of health, the impacts of systemic structures on health outcomes, and the broad policy and community changes needed to achieve health equity.
Manages programming communication and scheduling with internal and external stakeholders.
Prepares presentations, trainings and other materials as needed or directed.
Assists with development of processes, policies, and procedures.
Assists with identifying and proposing new community health projects.
Assists senior team members with identifying workshops on social determinants.
Convenes and lead workgroups at request and direction of the Director or Deputy Director.
Collaborates with national partners to acquire and share resources used in preventing and reducing health inequities.
Assists ensuring equity within all partnership development and retention efforts.
Provides support to the BCPHE office on other assigned tasks as needed.
Serves as a designated Bureau representative at internal and external meetings, events, and convenings in the absence of bureau leadership, providing updates, responding to stakeholder inquiries, and documenting follow-up actions.
Plans, coordinates, and facilitates internal and external meetings, including agenda development, stakeholder preparation, facilitation, documentation of decisions, and post-meeting action tracking.
Manages inventory and tracking systems for bureau resources, materials, and outreach assets, including forecasting needs, maintaining records, and advising on coordinating replenishment or procurement as needed.
Leads interagency communication efforts by coordinating information flow, deadlines, and deliverables across LDH offices and external government partners to support shared initiatives.
Serves as a primary point of contact for external stakeholders and community partners, managing ongoing communications, responding to inquiries, and ensuring timely follow-up on commitments.
Represents the Bureau at community events, conferences, health fairs and partner meetings, including tabling and communicating information about department programs and initiatives, and reporting back key insights to bureau leadership.
Develops and maintain standardized tools, templates, and documentation to support consistent meeting facilitation, partner engagement, and internal workflows.
Independently assesses emerging issues, partner needs, or operational gaps and recommend process improvements or engagement strategies to bureau leadership.
Provides informal guidance and coordination support to junior staff, bureau teammates, interns, fellows, or contractors involved in the bureau's activities.
Other tasks as directed.
QUALIFICATIONS
REQUIRED:
Bachelor’s Degree, or Associates degree with 3 years professional experience, or 6 years professional experience in lieu of the degree.
Minimum 3 years professional experience working with community partners and/or on community engagement efforts.
Minimum 1 year of professional experience with state health departments or state health systems.
Excellent analytical skills, effective organizational and time management skills.
Great attention to detail and follow up.
Ability to manage projects, assignments, and competing priorities.
Proficient in the use of virtual meeting technology and Microsoft Office, including but not limited to Outlook, Word, and Excel.
DESIRED:
Advanced degree.
Minimum 4 years professional experience working with community partners and/or on community engagement efforts.
Minimum 2 years professional experience with state health departments or state health systems.
Minimum 2 years professional experience with facilitating meetings and giving presentations.
Minimum 2 years professional experience with project management software (Monday.com or similar).
